Dear West Plano Pediatrics patients and families:
It is with mixed emotions that I announce my retirement from active practice, effective March 31, 2024. Since founding West Plano Pediatrics in 1995, it has been my honor to care for your infants, children, and adolescents. Thank you for placing your trust in me. I will always cherish the relationships that have been established throughout the years.
The ongoing well-being of my patients is very important to me. Rest assured that your child will continue to receive exceptional care at West Plano Pediatrics.
As of April 1, 2024, you may request care from any of our providers, which include Drs. Berger, Mix, Gair, and Reed. Dr. Rixney Reed has the most availability and has been with West Plano Pediatrics since 2022. Dr. Reed is a well-trained graduate of Louisiana State University Health Science Center Medical School. She completed her internship and residency at LSU as well.
If you choose not to stay with West Plano Pediatrics as your child’s medical home, I recommend looking for a new physician as soon as possible. You may contact the Collin County Medical Society to help you begin your search by providing the names of doctors in the area who are accepting new patients.
Your child’s medical records are confidential and will be secured at West Plano Pediatrics. A summary of medical records can be transferred to another doctor or released to you or to another person you designate only with your permission. If you choose to relocate to another practice, please sign a release of medical records form. You can find the form on our website here. Once we have received your signed form, my office will transfer your records to your new physician. If you plan to continue with West Plano Pediatrics, simply notify our staff which primary provider you have selected at the time you arrive or when you make your next appointment. At this time, your records will continue to be on file at West Plano Pediatrics.
Again, thank you for sharing a part of your family with me.
Best wishes for your future health.
Sincerely,
Elizabeth Y. Leon, M.D.
